Routine and expedited outpatient referrals

Use the referral form below, or call the academic office. The office will contact you and the patient to arrange a clinic appointment.

Urgent inpatient transfers

Do not use the web form for an urgent transfer. Call the UCI Health Transfer Center, which is staffed 24 hours a day, or submit through the transfer portal.

    Revision spine surgery

    Persistent pain after prior surgery, pseudarthrosis, adjacent segment degeneration, hardware failure.

    Spinal deformity

    Adult degenerative scoliosis, flatback syndrome, kyphotic collapse after osteoporotic fracture.

  • 03

    Spine trauma

    Unstable fractures, spinal cord compression, fractures through an ankylosed spine.

  • 04

    Tumor and infection

    Metastatic cord compression, pathologic fracture, spinal infection.

For an urgent inpatient transfer, call the Transfer Center at (714) 456-2222 instead of using this form.
